1. Four rules of upkeep

Rule 1 — the core: neuro-immune and endocrine-metabolic control. A younger brain really does track longer life. In July 2025 Stanford researchers reported in Nature Medicine on about 45,000 people: a “young” brain and immune system together were linked to about 56% lower death risk (about 40% for the brain, about 42% for the immune system). The brain is the command post — brainstem circuits for breathing and circulation; the HPA axis for stress, metabolism, immunity, heart and reproduction; neuroendocrine and autonomic control of circadian rhythm, energy balance and chronic inflammation. It is the console of a youthful state.

Rule 2 — the gut as engine. Work in Nature found centenarians enriched for a distinctive flora that makes unusual bile acids, which may restrain gut pathogens. Honda’s group at Keio compared 160 centenarians, 112 people aged 85–89 and 47 aged 21–55, and found those bile-acid pathways richer in the oldest. Beneficial species dominate in long-lived guts; poor diet unbalances them. Bifidobacteria have been reported on the order of 100-fold higher in centenarians than in ordinary older adults, and about 50-fold higher in healthy older adults than in those who are ill. Gut age tracks health.

Rule 3 — environment, the neglected driver. Radiation, air pollution, pathogens, chronic stress, lost sleep, microplastics, too much or too little sun all add cell injury. Prevention-first longevity care weighs information against cumulative harm — including in how imaging is chosen — rather than treating every scan as free.

Rule 4 — the body speaks early. Poor appetite, poor sleep, a slower walk, sluggish bowels, a restless mind, worse adaptation to the environment: not merely “sub-health”, but early ageing signals. The earlier they are read, the more room there is to bend the curve.

2. How a longevity service runs

A pattern taking shape is a specialist assessment centre plus long-term management: measure ageing, write a plan, monitor across the life cycle, act before disease.

1. A personal longevity file: standard check-ups, deep questionnaires, functional and molecular tests, immune/gut/metabolic/inflammatory/brain markers, and a pace-of-ageing estimate — longevity assessment and diagnostics.

2. A personal prescription: food and lifestyle, plus cell-level support, the microbiome, training, and control of environmental injury. The point is a younger functional state, not only a later disease.

3. Follow-up: not a single procedure but monthly tracking, adjustments, and a yearly re-score of how fast ageing is running. The clearest value is a measurable change in biological age or pace.
